Input format for Unif.exe

1. With frequencies: should have four columns separated by spaces
First column: an integer 1 to "Number of Treatments" denoting the number of the treatment
 group of the observation
Second column: cluster size of the observation
Third column: number of responses in the cluster
Fourth column: the frequency with which such a combination occurred

2. Without frequencies: should have only the first three columns of the "with frequencies"
version

3. Dose levels should be separated with semicolons(;) with a semicolumn at the end.
For example, for four doses we could have: 0;1;2;3;  
The number of doses should coincide with the number stated as the "Number of Treatments".
Note that the doses are used only for the Rao-Scott statistic, 
but something has to be entered even if only other statistics are of interest.
