When coming to your doctor appointment, please bring the following:

[ ] referral or authorization documents
[ ] co-payment if applicable
[ ] your medical information and reports from other hospitals or clinics. You will need to request these from the facility where the tests were done or from your primary care physician. These would include:

• test results • lab reports • medical history charts • other information you have pertaining to this illness [ ] Actual films, X-rays or CDs (rather than written reports) of the following: • PET scans • CT scans • ultrasounds • X-rays Huntsman Cancer Institute doctors have access to results of tests performed at University of Utah Hospital or Huntsman Cancer Institute. Just inform your doctor were these tests were performed.

[ ] a list of all your medications including herbs and dietary supplements, along with their dosages
[ ] a list of your food allergies including type of reactions
[ ] a phone number where you can be contacted
[ ] questions you may have for your doctor
[ ] appointment book or calendar
